Log into your Synchroteam.com account
Click on Configuration, choose Reports
Click on Add a report template
Then Edit the report template created
Create information fields that you wish to capture on field organized in Family/Items
Each field can be: check box, picklist, free text, mandatory or conditional
Reports can be duplicated
